NEW PATIENT

LOCATION

If this is your first visit to Herbs and Healing/Acupuncture Associates, here are a few things to keep in mind:

To schedule an appointment, please call 212.777.3909

Please download our new patient forms AND fill these out before you come in for your first visit. This will save time and make your first visit more efficient. If you need to download Adobe Acrobat click here.

Your first visit can take anywhere from one to two hours, so please plan accordingly. If you are having an acupuncture/acupressure treatment, it’s a good idea to eat something—if even just a small snack—an hour or two before your appointment.

If you have any information related to your problem like reports from lab work, X-rays or MRIs please bring copies with you. Also please make a detailed list of all medications and nutritional supplements and herbs you are taking.

We have gowns and towels for you to use during your treatment, but some patients are more comfortable wearing loose gym clothes. While it is not necessary, you may want to wear loose clothing (or bring a change of clothes).
